This sweep reviewed the open-issue inventory and closed 46 issues with verified completion, supersession, or duplicate evidence. It marked one unresolved operations report as stale for owner review. No code, branch, or production changes were made.

Outcome Issues Count Basis
Closed as done, superseded, or duplicate #4, #5, #22, #24, #46, #48, #56, #69, #70, #73, #76, #78, #80, #81, #101, #144, #154, #171, #174, #193, #199, #216, #222, #226, #227, #257, #266, #294, #299, #302, #304, #306, #316, #317, #335, #337, #349, #358, #379, #387, #389, #395, #520, #521, #643, #647 46 Evidence or replacement references were added in each closing comment. #144's SECURITY-marked fix and regression test are on origin/dev. #349's issue text and prior comment were neutralized before closing under DESIGN §43.
Marked stale; left open #26 1 The reported incident was recovered, its root cause is unknown, and the old condition is not reproducible from current code. The issue is marked “STALE?” for owner review.
Commented; left open #41, #55, #71, #93, #223, #249, #256, #300, #330, #719, #866 11 Each has remaining work, an unverified target, an explicit OPEN/LATER status, or an incomplete audit scope. #223 and #256 were reopened after the marker audit found their explicit OPEN status; #249 was reopened because it is marked LATER.
Other issues left open All remaining original open issues (639) 639 No safe completion or supersession basis was established in this pass. Full issue records and comment histories were fetched, but descriptions and evidence for this entire remainder were not individually validated.

Known partials: #719 remains open because its import-specific replacement does not cover unresolved imported links and identity, and corpus/channel coverage is incomplete. #643 and #647 were superseded by #940; that replacement remains queued, so the work is not reported as landed. Measurement goals in #71 and #330 remain unverified.

The inventory had 697 original open issues before dispositions. The original-issue counts above sum to 651 remaining open issues after 46 verified closures. The report issue itself adds one more open issue. A read-only API pass fetched 650 then-open issue records (including this report) and 4,694 comments; a status-marker check of all 48 initial closure candidates caused #223 and #256 to be reopened. No issue was closed when its evidence was uncertain. No secrets or financial data were added to this report.

1–340 issue hygiene report

Completed the individual review of every issue in #1–#340 that was OPEN at the start. I checked each against origin/dev code, tests, design decisions, and the paused merge queue. The initial inventory had 96 open issues: 7 are now closed with evidence, and 89 remain open. No code changed. Branch dev remains at head d4e7188810a89fb0e8e6b162279917f7e23989f9; no commit, push, merge, or deploy was made.

Outcome Issues Count Evidence / reason
Closed #35, #37, #202, #228, #251, #271, #274 7 Each closing comment cites the applicable landed merge/fix and current test or evidence. #274 is superseded by #275.
Commented; open #8, #90 2 #8: DESIGN §17 now records the managed key, but PARITY.md still reports 387/398 tests ported (11 remain). #90: TypeScript/Rust anchor handling exists, but anchored quote/callout vectors and the recorded real-client resolution check are missing.
Stale; open #26 1 The 2026-09-24 host incident was recovered; its root cause is unknown, and the historical host-state condition has no reproducer in current code. The issue remains titled STALE? for owner review; evidence was added.
Open: future, owner decision, or not built #25, #34, #49, #50, #67, #91, #92, #196, #200, #240, #241, #249, #293, #296, #297, #300 16 Explicit future/later/open decisions or required owner/infra work remain.

Security/BLOCKER issues remain open unless their fix is verifiably on dev.

Gates: No code changed, so Rust and web gates were not run. Cleanup output from cargo clean (verbatim): Removed 1 file, 356B total. apps/web/build was absent.

Decisions: No new product or design decisions were needed. I left explicitly future, queued, partial, security, and uncertain items open under the issue rules.

Issue hygiene report — #681–#1010

Branch/base/head: dev at d4e7188810a89fb0e8e6b162279917f7e23989f9; origin/dev matched. The worktree is clean. No source files changed and no commit was made.

I viewed each existing issue in the range individually. #1010 does not exist. The source diff from production commit c4a61e8cf090170f35b1bed3350d9de20c83ecd5 to origin/dev contains only deploy/media-sandbox, docs/DESIGN.md, and tests/adversarial/prepare-media-runtime.sh; it contains no product source changes. I cross-checked closure candidates and active findings against their current source/test paths. I closed no issue as fixed because no fix was verified on origin/dev.

Disposition Count Result
Closed as duplicate 2 #1006 → #958; #994 → #716. The canonical issues remain open.
Commented and left open 7 #719, #963, #965, #1000, #1003, #1004, #1009. Each comment records current source evidence and what remains.
In paused merge-round-7b queue; left open 122 Their branch work is not on origin/dev. #1000 is also in the commented row.
Other open issues; left open 197 No verified current-dev completion, owner supersession, or duplicate justified closure.
Stale-age candidates 0 No open issue had 7+ days without activity at the 2026-10-03 13:01 UTC check.
Already closed before this sweep 2 #709 and #763; skipped.
Not found 1 #1010.

The 325 open issues are 122 queued, 6 commented outside the queue, and 197 other open issues. The seven-comment count overlaps the queue at #1000. No Security or Blocker issue was closed.

The two duplicate decisions have current regression evidence:

  • python3 tests/adversarial/test_admin_classification.py confirms #994 duplicates #716. The scope guard still fails because the fixture omits Notes read/write/full.
  • node --test apps/web/e2e/harness.test.mjs confirms #1006 duplicates #958. Seven tests fail because the VM fixture has no window; one passes.

Focused test output:

Gates: Per-crate Rust gates and web checks were not run because this issue-only job changed no source code. cargo clean exited 0; output:

     Removed 1 file, 356B total

No apps/web/build or apps/web/.svelte-kit output existed to remove. Temporary issue inventory files were removed.

Known gaps: #716 and #958 still have failing focused tests and remain with their fix owners. The 325 open issues remain open as listed above.

Decisions: No design change or DESIGN-silent product decision was needed. Duplicate reports were closed against their canonical open issues; uncertain and queued issues remain open.

# Issue hygiene report — #681–#1010 **Branch/base/head:** `dev` at `d4e7188810a89fb0e8e6b162279917f7e23989f9`; `origin/dev` matched. The worktree is clean. No source files changed and no commit was made. I viewed each existing issue in the range individually. #1010 does not exist. The source diff from production commit `c4a61e8cf090170f35b1bed3350d9de20c83ecd5` to `origin/dev` contains only `deploy/media-sandbox`, `docs/DESIGN.md`, and `tests/adversarial/prepare-media-runtime.sh`; it contains no product source changes. I cross-checked closure candidates and active findings against their current source/test paths. I closed no issue as fixed because no fix was verified on `origin/dev`. | Disposition | Count | Result | | --- | ---: | --- | | Closed as duplicate | 2 | #1006 → #958; #994 → #716. Correction to the focused Python test output in my #681–#1010 report: its first line is .F............ (not .F.............). The copied unittest output has 14 test markers, with one failure. All other counts and dispositions remain unchanged.
